Medical Case Management
Medical case management is a core medical service in the Ryan White HIV/AIDS Program and aims to provide treatment and care for people living with HIV/AIDS by engaging, identifying and eliminating barriers to care.
Housing Assistance
The vision of Cempa Community Care’s Housing programs are to increase housing stability and improve the quality of life for the individual and for their families.
Transportation
Cempa’s Transportation Services provide assistance with medical appointment transportation services, bus cards or passes, and more.
Food Market
The Cempa Food Market Provides onsite nutritional assistance by distributing food boxes and pantry items for clients in need.
Nutritional Counseling Services
Work with Cempa’s Registered Dietitian to evaluate your nutritional needs and create individualized health goals based on medical history, developing issues, or new diagnosis.
