Cempa Community Care has partnered with Metropolitan Ministries and other community partners to increase access to health care, screenings, and education in the Chattanooga 37407 Area. 

The Cempa Community Care Impact Hub Clinic will offer a full range of services including Primary Medical Care, Women’s Health, Laboratory Services, and Health Maintenance for Preventative Care.

Treatment & Linkage To Care

Individuals who seek care at the Impact Hub will receive integrated health screenings for General Health, Mental Health and Syndemic Comorbidities such as HIV.

Following the combined health screening, clients may be recommended and provided prevention services including STEP TN Harm Reduction, PrEP, Prevention Education and Condom Distribution.

Individuals who receive a positive diagnosis during the combined health screenings will be linked to a pipeline of care and wrap-around services:

All individuals, regardless of diagnosis, will have access to Cempa’s Primary Care Services at the Impact Hub including referrals to mental health counseling through licensed clinical social workers, women’s health, nutritional counseling, and more.

Any individual in need of ongoing care due to a diagnosis will work with Case Management Staff to coordinate a comprehensive continuum of care that includes social support services and appropriate medical treatment.

Any individual who received a positive diagnosis of HIV will be assigned a Medical Case Manager to assist with ongoing treatment and adherence to the continuum of care. Individuals diagnosed with HIV will be provided annual screenings, medication adherence counseling, mental health counseling, nutritional counseling, and additional supportive services, such as assistance with housing, food, transportation, dentistry, and more.

If a client is newly diagnosed with or has Chronic Hepatitis C, Cempa will provide individualized health plans to treat and monitor the HCV infection. Cempa’s providers follow CDC guidelines to perform a medical evaluation and regular checkups, screen for and provide Hepatitis A & B vaccinations, test for and assess risk for HIV transmission, and discuss preventative care.

Individuals will also have access to nutritional counseling to help reach health goals as part of the HCV treatment plan.

Any client who receives a positive STI diagnosis will be referred to treatment either through the Impact Hub clinic, the Health Department, or their Primary Care Physician.

Any identified testing need that cannot be met onsite will result in a referral to the nearest and most appropriate resource in addition to linkage with a Cempa Community Care Case Manager to ensure follow-up.

The Impact Hub brings together multiple non-profit organizations to provide services within one central space to provide increased access to health care, health screenings, health education, and vaccinations. Referrals for substance abuse disorder, or other supportive services such as employment, housing, food, or mental health utilize current agencies within the Impact Hub, and with Cempa’s Community Partners.
