Work with Cempa Community Care’s Transportation Services Staff for assistance with medical appointment transportation services, bus cards or passes, gas cards, Uber Health Services, and food box deliveries.

To be eligible for Cempa’s Transportation Services clients must:

1. Client must have HIV (Human Immunodeficiency Virus) or AIDS (Acquired Immunodeficiency Syndrome) and be Medically Case Managed through the Ryan White Part B Program

2. Client must also be Case Managed at Cempa Community Care

3. Total household gross income must be less than 400% of Federal Poverty Level ( FPL) guidelines.
